Get a Library Card by Mail
If you have registered for a class or workshop through The Center in the last two years, you can apply for a library card online.
If you have not registered for a class or workshop through The Center in the last two years, you can apply for a library card via mail by downloading and completing the PDF form below.
To apply for a library card, mail the form along with a photocopy of
- 1) your driver's license or state ID (SSN crossed out), AND
- 2) proof of employment in education, such as a recent pay stub, or your employee ID card.
Mail the form and photocopies to The Center, Attn: The Center Library, 2626 S. Clearbrook Drive, Arlington Heights, IL 60005.
